Transcripts
If you need a copy of your "Official Transcript" sent, you must request this in writing, enclosing a check for $10.00 (per transcript) payable to "LA BioMed".  You may also request these by email at:  lshort@labiomed.org please be sure that you include the address(es) you want the transcripts sent to.

Licensure
Any Furnishing Number applications can be downloaded from the BRN at http://rn.ca.gov/pdfs/applicants/npf-app.pdf (file: npf-app.pdf).  Fill out the entire form then send it to us along with a check to "BRN" for $50.00 and a check to "LA BioMed" for $10.00.  We will complete your application and send it to the BRN for you.  For your information:
